Making an application

​All applications to SACAT are made online. As part of the state transition to digital-by-default, paper forms are not available.

What fees are payable?

With the exception of some community stream matters, most SACAT applications attract a fee. Fee rates are available here. If you are seeking a fee waiver or concession, please upload a copy of your concession information at the end of the application.

What documents do I need to provide?

Before starting your application you will need to supply the required documentation based on the application type. Your application may not be listed by SACAT until all documentation is received.

Guardianship, administration of a person's affairs, mental health, advance care directive or consent to medical or dental treatment applications

Professional disciplinary applications

  • Please scan and upload a copy of your grounds, particulars or facts of the complaint.
  • Please scan and upload any further supporting documentation you have available at the time of lodgement.
